The diary is a "little book, a brick of bible-thin pages bound in soft black leather". There is a pen clipped inside the spine, so he's never short of one.
Todd has trained himself to write in it every night (no matter how tired), making notes that he might need in order to know where he is in case he wakes up as a younger self. One of the most common uses is for recording the name of whoever he's staying with after he hooks up with someone in a bar. He feels that in order to avoid suspicion he needs to at least know a name, what attracted him to this guy, and if it meant anything to him.
